If you are a healthcare professional and would like to refer your patient to one of our clinical services, please download a form, complete and send back to us. Our clinical services include our Palliative Care Specialist Nurse team, Virtual Ward, Inpatient Unit, Day Services, consultants, physiotherapy, occupational therapy and complementary therapy.
Please note that patients referred to our services need to be registered with a GP in North Derbyshire.
If you need to speak to someone urgently about a referral, please call 01246 565 026.
Please note that referrals for our complementary therapy services are paused for the foreseeable future.
