Shopping with us or donating items for us to sell helps fund our compassionate care.
If you feel your patient may benefit from our services, you will need to make a referral to us.
The information below is for healthcare professionals. If you are a patient or loved one and would like to access our services, please speak to a healthcare professional involved in your care, for example, your GP or district nurse.
Important information for referrers: Please ensure that you choose the right service for the referral you wish to make.
Please note that referrals for complementary therapy and counselling services are paused for the foreseeable future.
Download the referral form for the service you wish to make a referral to. Complete the form and send it to us by email or post. Any incomplete forms may result in a delay. Carefully read the referral criteria to ensure the service is appropriate for the patient. Please ensure that the patient has consented to the referral and the sharing of their clinical records with Ashgate Hospice.
Once we have received your referral, we may contact you, as the referrer, to get more information if required. Please ensure your contact details are up to date to avoid any delays.
If your referral meets our criteria, we will then contact the patient or their next of kin to carry out an assessment. This will help to identify the patient’s needs and if they have any concerns that we can help with. We may also be able to signpost or refer to other services if needed.
This step will not be taken for referrals to our Inpatient Unit.

The purpose of a Virtual Ward is to provide safe care in the patient’s own home (including care homes) for palliative patients.
The aim is to avoid admission into acute hospitals where appropriate or support faster discharges back home. This will be facilitated using face-to-face visits and the use of digital technology.
